This Privacy Policy is meant to explain what personal information Fountain Health collects, why we collect it, how it is processed, and how you can update, manage, export, and delete your information.

You directly provide Fountain Health with most of the data we collect at the time you make an initial appointment with us. We also collect and process data when you:

  • Complete and submit a new patient information form

  • Consent to have medical tests done as recommended by our physicians

Fountain Health may also receive your data indirectly from the following sources:

  • Your referring doctor or general physician

  • Laboratories where you have completed blood, urine, and other medical tests

  • Hospitals where you have undergone procedures

  • Other doctors or specialists you have visited for treatment

Fountain Health collects your data so that we can:

  • Provide you with health care and treatment

  • Contact you regarding appointments, lab tests and referrals, etc.

How do we store your data?

  • Fountain Health securely stores your data at 63 Victoria Street, Hamilton HM 12, Bermuda where our building is monitored by an alarm system.

  • Personal data is stored digitally in our secure, password protected database and password protected files.

  • We also manage the access of these files.

  • We retain data for 7 years after a patient is deceased or left the practice, as required by law.

  • Hard copies of personal information and lab results are destroyed once a month at the Tynes Bay Waste Management Facility.

 

What are your data protection rights?

Fountain Health would like to make sure you are fully aware of all your data protection rights. Every patient is entitled to the following:

 

  • The right to access - you have the right to ask Fountain Health for copies of your personal information. This right always applies. We may charge a small fee for this service.

  • The right to rectification - you have the right to ask Fountain Health to correct any information you think is inaccurate. You also have the right to ask us to complete information you believe is incomplete.

 

  • The right to erasure - you have the right to ask us to erase your personal data, under certain circumstances.

 

  • The right to restrict processing - you have the right to request that Fountain Health restrict the processing of your personal data in certain circumstances.

  • The right to object to processing - you have the right to object to Fountain Health’s processing of your personal data, under certain conditions.

 

  • Your right to data portability - you have the right to request that Fountain Health transfer the data that we have collected to another organization, or directly to  under certain conditions.
